Cheap multis use the inactive forms (cyanocobalamin, folic acid) that require liver conversion. The active forms (methylcobalamin, methylfolate) skip that step entirely.
Roughly 40% of the US population carries an MTHFR gene variant that reduces methylfolate conversion efficiency by 30-70%. For these individuals, supplementing with folic acid actually accumulates unmetabolized folic acid in serum without raising functional methylfolate. The active forms bypass this entirely.
